What is COPD? Early Signs You Should Not Ignore

COPD is one of the leading causes of breathlessness in adults — and early diagnosis makes a life-changing difference.

Chronic Obstructive Pulmonary Disease (COPD) is a progressive lung condition characterised by persistent airflow limitation and difficulty breathing. It is one of the leading causes of chronic illness and death worldwide, yet a large proportion of patients remain undiagnosed until the disease has significantly advanced.

The good news is that when detected early, COPD can be effectively managed — significantly slowing its progression and improving quality of life.

What Exactly is COPD?

COPD is an umbrella term that includes two main conditions:

  • Chronic Bronchitis — persistent inflammation and narrowing of the airways, causing a productive cough lasting at least three months for two consecutive years
  • Emphysema — destruction of the air sacs (alveoli) in the lungs, reducing the surface area available for oxygen exchange

Most patients have a combination of both. Unlike asthma, COPD involves permanent structural changes to the lungs and airways.

Who is at Risk?

  • Smokers and ex-smokers — cigarette smoking remains the single greatest risk factor
  • Non-smokers exposed to biomass fuel — cooking or heating with wood, dung, or crop waste indoors is a significant risk, particularly among women
  • Occupational exposure — prolonged exposure to dust, fumes, chemicals, or mining environments
  • Air pollution — long-term exposure to outdoor particulate pollution
  • Genetic factors — Alpha-1 antitrypsin deficiency is a rare hereditary cause
  • History of childhood respiratory infections

Early Signs of COPD You Should Not Ignore

COPD develops gradually over many years. The early symptoms are often dismissed as a normal part of ageing or attributed to smoking. This is why so many cases go undetected.

1. Chronic Cough

A cough that persists for weeks or months, with or without mucus production, is never truly "normal." This is often the first sign of COPD and is frequently labelled as a smoker's cough.

2. Increased Mucus Production

Producing more mucus than usual — particularly in the mornings — is a common early symptom of chronic bronchitis.

3. Shortness of Breath on Exertion

Feeling breathless during activities that previously caused no difficulty — such as climbing stairs or walking uphill — is an important early warning sign. Many patients adapt by simply doing less.

4. Frequent Chest Infections

Recurrent respiratory infections that take longer than usual to resolve can indicate underlying COPD.

5. Wheezing

A whistling or squeaking sound when breathing, especially during exertion.

How is COPD Diagnosed?

The gold standard for diagnosing COPD is spirometry — a simple, painless breathing test that measures lung function. A spirometry result showing airflow obstruction that does not fully reverse with bronchodilator treatment confirms the diagnosis.

Additional investigations may include:

  • Chest X-ray or CT scan
  • Arterial blood gas measurement
  • Six-Minute Walk Test (6MWT) to assess exercise tolerance
  • DLCO (Diffusion Lung Capacity) for assessing gas exchange

Can COPD Be Treated?

While COPD cannot be reversed, it can be effectively managed. Treatment aims to reduce symptoms, slow progression, and prevent complications:

  • Smoking cessation — the single most important intervention at any stage
  • Bronchodilator inhalers — to open the airways and improve breathing
  • Pulmonary rehabilitation — to improve exercise capacity and quality of life
  • Vaccinations — flu and pneumococcal vaccines to prevent infections
  • Home oxygen therapy — for patients with low oxygen levels
  • Long-term follow-up — to monitor progression and adjust treatment
